DOE contractor or subcontractor employees or employee representatives may submit a request to the Department of Energy’s Office of Enforcement, within the Office of Enterprise Assessments, to initiate an enforcement investigation or inspection into an alleged violation of DOE safety or security regulatory requirements (10 C.F.R. Parts 820, 824, 830, 835, 850, and 851). Additional information on the Request for Investigation (RFI) process can be found in Chapter V of the Safety and Security Enforcement Process Overview.

Do not use the form to report an emergency. To report an emergency, fatality, or imminent life-threatening situation, contact your supervisor and local DOE officials using locally established procedures.

After a Request for Investigation form is submitted electronically, the Confirmation page will assign a Unique Identifier Number to the request. Please maintain this Unique Identifier Number, and refer to it during any contact with the Office of Enforcement about your request.

WHISTLEBLOWER COMPLAINTS

DOE contractor or subcontractor employees who wish to file a complaint related to reprisal or retaliation (i.e., whistleblower) can pursue their claim through either the DOE Contractor Employee Protection Program (10 CFR Part 708), which is administered by the Office of Hearings and Appeals, or through the Department of Labor’s Whistleblower Protection Program (29 CFR Part 24). Either option is available to DOE contractor employees to pursue for remedies for retaliation for raising safety concerns at DOE facilities. If safety-related retaliation is determined to have occurred, then the Office of Enforcement may elect to pursue enforcement action against the contractor.
