Preliminary Notice of Violation, Lockheed Martin Energy Systems - EA-2000-11

Issued to Lockheed Martin Energy Systems, related to Quality Assurance Issues at the Y-12 National Security Complex, (EA-2000-11)

On August 25, 2000, the National Nuclear Security Administration issued a Preliminary Notice of Violation (EA 2000-11) to Lockheed Martin Energy Systems for violations of 10 C.F.R. 830 associated with multiple issues at the Y-12 site including (1) deficiencies in the design, procurement and fabrication of a critical radioactive material supply system, (2) an unanticipated event in the Alpha 5 Facility that resulted in 11 workers being injured, (3) violations of criticality safety requirements and work process controls in Building 9212, and (4) violations involving Operational Safety Requirements and other Authorization Basis requirements.
