Status and Effectiveness of DOE Efforts to Learn from Internal and External Operating Experience in Accordance with Commitment #20 of the DOE Implementation Plan for Defense Nuclear Facilities Safety Board Recommendation 2004-1

This report provides the results of an effectiveness review of the Department of Energy's (DOE) Action Plan for improving its operating experience program and learning lessons from the Columbia space shuttle accident and Davis-Besse nuclear power plant event.  The review was performed by the DOE  Office of Health, Safety and Security and fulfills a commitment of the DOE Implementation Plan to Improve Oversight of Nuclear Operations, which was developed in response to Defense Nuclear Facilities Safety Board Recommendation 2004-1, Oversight of Complex, High-Hazard Nuclear Operations.
