Review of Implementation Verification Review Processes at the Idaho Site - Phase 1 of 2

This report provides the results of the first phase of an independent oversight review of the implementation verification review (IVR) processes at the Department of Energy's (DOE) Idaho Site.  The review was conducted September 12-23, 2011, by the Office of Safety and Emergency Management Evaluations, within the DOE Office of Health, Safety and Security, and was carried out within the broader context of an ongoing program of assessments of the execution of IVRs at DOE sites with hazard category 1, 2, and 3 nuclear facilities.  The purpose of the review was to evaluate, in two phases, the processes and methods used at the Idaho Site for verifying the implementation of new or substantially revised facility safety basis hazard controls.
