Hanford Waste Treatment and Immobilization Plant Low Activity Waste Melter Off-gas Process System Hazards Analysis Activity Observation [HIAR-WTP-2013-05-13]
This Independent Activity Report documents an oversight activity conducted by the Office of Health, Safety and Security’s (HSS) Office of Safety and Emergency Management Evaluations from May 13 – June 28, 2013, at the Hanford Waste Treatment and Immobilization Plant (WTP). The activity consisted of HSS staff observing a limited portion of the start of the hazard analysis (HA) for WTP Low Activity Waste (LAW) Primary Off-gas System. The primary purpose of this HSS field activity was to observe and understand the revised HA approach implemented by Bechtel National, Inc., the contractor responsible for the design and construction of WTP for the Department of Energy Office of River Protection. A secondary purpose was to understand the design and potential failure modes of the LAW melter process and associated off-gas systems.
